Well lets see....

1.) Ran out iof ammo, took out my magizine threw it at the guy, then threw the gun at him since he wouldn't stop shooting.
2.) I tryed to ambush someone with my airsoft claymore, but then shot it and it happened to be empty.
3.) Left my old KJW mark 1 on the grill behind my house while my dad was out there...BAD MOVE..I walk out only to hear the noise of gas filling up the magizine...bam i got shot in the head chest and genitals.
4.)Tryed to make a airsoft grenade..loaded it and it blew up in my face.
5.) Tryed to use cardboard as a sheild...HURT alot
6.) I had a shot at someone.. tryed to be a cool sniper and ask for confermation and he heard me and shot me in the face

Hmnmmm. I haven't made any big mistakes. But I have seen people make really bad choices.

I had a teammate fail to reload before charging the enemy. Shot about 8 rounds, then was out in a hailstorm of plastic.

Also, I have had a team mate who's shoes came untied, and then he decided to bend over and tie them. That is when 4 of the OPFOR came out of the woodwork and took him and 2 of my other guys.

Ive had it where other players on my team wuss out quickly in a game if they think they are surrounded or something similar. They will hear a few rounds hitting around them, and they will just surrender. But then I have other teamates who will sit somewhere and refuse to leave the area no matter how many bbs are swarming there. So I get the worst of both worlds.

Took a broken gun to a war, and hoped, that by the grace of god it would somehow start working when the SHTF. It didn't. It was an ICS MP5A5, I later found out that the only problem was that the tappet plate and nozzle weren't connected, I'm retarded.
